Mix Up In Medical Order Leads To Death Of A Veteran [VIDEO]

A terrible mistake made by some nurses in an Ann Arbor hospital lead to the death of veteran after open-heart surgery. Nurses thought that the patient had a "do not attempt to resuscitate" order in the chart. 

Sadly, that was not the case and the veteran ended up dying from a heart attack after surgery. An investigation also revealed that this patient was not on a heart monitor like he should have been.
